Your first step in becoming parents is to schedule an appointment with Dr. Brandeis. Dr. Brandeis has 20 years of experience and is experienced in the very latest in fertility technologies.

At your first appointment, we encourage your husband to attend. You will meet with Dr. Brandeis and your nurse coordinator.

Dr. Brandeis will review your medical history and records and share his thoughts based on this preliminary information. He may also use an ultrasound to visualize your ovaries and uterus.

We encourage you to print out :

• Patient Intake Form
• Wife Questionnaire
• Husband Questionnaire

And please bring these forms with you for your first visit.

Your past records are helpful: The following documents, if available, should be brought with you to your first appointment:

• Estradiol, FSH, LH testing
• Hysterosalpinogram
• Transvaginal Sonogram
• Sonohysterogram
• Pelvic MRI
• Sperm analysis
• IVF records – in particular:
          - Your stimulation page which usually summarizes the medications and the             sonograms
          - Embryology Records
